My copy arrived today. I follwed the instructions posted
previously and sent them $5.95 plus $3 Postage and Handling. I
used the following address and had no problems:

Falls Village, CT 06031
On their receipt, they list their address the same as above,
except they list ZIP+4: 06031-5000. For anyone who may have
had a problem with USPS, perhaps the full zip will help.
In any case, $8.95 for the book seems like a real bargain.
