Nepenthes Cuttings Question

I am going to be taking some cuttings from several Nepenthes spp. and
although I have grown several, have not actually propagated them myself.
I have a few questions:

1) In the experience of people here who have propagated from
cuttings, which roots better, the tip cutting or the ones farther down?

2) Do you use a hormone? (I plan to)

3) I have read that if I take cuttings off upper growth, all growth
thereafter will be upper growth as well. Makes sense. But I have also
heard that there is a way to get them to produce lower pitchers again.
Will the axilarry buds on upper growth produce lower pitchers?

4) Any other hints?
